はじめに

　多嚢胞性卵巣症候群（polycystic ovary syndrome : PCOS）患者における糖尿病・循環器疾患の長期予後に関して，対照群よりも高い罹患率を示す報告が増えている．それに伴い，PCOSをメタボリックシンドロームの一表現型，あるいは類縁症候群として捉える傾向が強まっている．これはSteinとLeventhalにより1935年に生殖器系の症候群としての概念が提唱されてから70年を経て，PCOSをより広範囲に，全身臓器に影響する症候群の一部として捉え直すという，大きなパラダイムシフトである．

　本邦のPCOS診断基準が，高アンドロゲン血症を重視する欧米のPCOS診断基準とは様相を異にするのは，高アンドロゲン血症の占める割合の多寡という患者集団における人種間の差を根源としている．しかしながら，人種間による症状の特徴的な差異も表現型の多様性として認識し，多臓器症候群として探求することは，PCOSの病因を探るうえできわめて有意義である．

　本稿ではPCOS患者の糖尿病・循環器疾患の長期予後を，その病因論も含め，インスリン抵抗性に基軸を置くメタボリックシンドロームと関連させて概説する．
