子宮体癌の手術療法

はじめに

　子宮体癌の手術療法の術式とその適応を臨床的エビデンス^1）より根拠を述べる．

初期体癌

　体癌の80％はI～II期であり，腟式，腹腔鏡での手術があるが，腹式子宮全摘出術（TAH，I期は単純，適宜準広汎，II期は広汎）＋両側卵管卵巣全摘出（BSO）と手術的病期診断が標準治療法である^2）．手術で90％の生存率が得られる．高リスク群では局所，遠隔再発が起こり，子宮摘出±放射線療法は放射線療法単独より予後がよい．
