5.卵巣癌の腹腔内抗癌剤投与法

はじめに

　卵巣癌に対する標準的化学療法であるタキサン系抗癌剤と白金製剤との併用療法は高い奏効率を示し，その予後を改善した^1）．しかしながら，長期予後は未だ満足できるものではなく，種々の試みがなされている．なかでも，抗癌剤の投与経路については多くの検討がなされてきた．

　卵巣癌は早期から腹腔内播種を起こすことから，播種病巣の制御は予後を決定する重要な因子である．卵巣癌に対する腹腔内抗癌剤投与（intraperitoneal chemotherapy）の利点としては，（1）卵巣癌の進展形式と同様の経路で薬剤が分布する可能性がある，（2）高濃度の薬剤を播種病変に直接接触させ得る，などがある．しかしながら，卵巣癌に対する腹腔内抗癌剤投与に関して多くの研究がなされてきたにもかかわらず，未だ最終的なコンセンサスが得られていない現状にある．

　本稿では，卵巣癌に対する腹腔内抗癌剤投与の最新知見と問題点について概説する．
