1．排卵誘発法の変遷

はじめに

　排卵誘発は，排卵のない患者に対して排卵を誘発するovulation inductionと，排卵を有する患者に対し複数の排卵を誘起し妊娠率の向上を期待するsuper ovulationまたはcontrolled ovarian hyperstimulation（COH）に分けられる．

　1960年代に入りクロミフェン，ゴナドトロピンによる排卵誘発の臨床応用が進み，不妊患者に対して多くの福音がもたらせられた．1980年代に入り早発LHサージ防止薬としてGnRHアゴニストが実用化され，また，1990年代の中期にはGnRHアンタゴニストが使用可能となり，GnRHアナログを併用したゴナドトロピンによる排卵誘発が体外受精を中心に排卵誘発法として汎用されるに至っている．また，遺伝子工学の進歩により，最近多くの遺伝子組み換え型ゴナドトロピン製剤が開発されている（図1）．

　排卵誘発には良好な排卵率，妊娠率と低侵襲，低コスト，簡便性のほかに多胎妊娠，卵巣過剰刺激などの副作用を極力抑えることが要求される．近年，多胎妊娠を防止するための種々の排卵誘発法の工夫がなされるようになった．多胎妊娠防止を目的とした排卵誘発の工夫に関しては他稿を参照いただき，本稿では排卵誘発の薬物療法の変遷に関して概説する．
