2．余剰胚凍結法と凍結胚移植周期の管理

余剰胚凍結保存法についての概略

1. はじめに

　ヒト生殖補助医療（ART）において，余剰胚の凍結保存は重要な治療技術の1つであり，現在ではさまざまな凍結法が臨床的に用いられている．その理由としては，体外受精で得られた胚のうち，新鮮な状態で移植したあとの余剰胚を凍結保存しておくことにより，その後の周期で融解後の生存胚を少数ずつ移植し妊娠に向けることができるためである．凍結胚の利用により，採卵を毎回行う必要がないことから，患者の負担が軽減され，採卵周期当たりの妊娠率を向上させることができる．そのうえ1回の移植胚数を減らすことで，多胎の防止にも役立ち，子宮内環境不良やOHSSの発症・増悪が考慮される場合などの新鮮胚を移植することが不適当な場合はすべての胚を保存し，その後の自然周期または子宮内膜作成周期で移植することも可能となる．
