2.抗リン脂質抗体陽性の不育症の治療は？

1 はじめに

　2006年2月に，新しい抗リン脂質抗体症候群の「分類基準（classification criteria）」が発表された^1）．「分類基準」とは，一般に膠原病などの診断に用いられているクライテリアで，症候群（syndrome）であるがゆえに，あえて「診断」ではなく「分類」という単語を用いている．これは，今後の医学の進歩により随時変更が加えられていくことを意味している．抗リン脂質抗体症候群の「分類基準」には，臨床所見と検査所見が示してあり，少なくとも1項目ずつ認めれば抗リン脂質抗体症候群といえる．表1に産科的臨床所見を，表2に抗リン脂質抗体検査所見を抜粋した．ただし，臨床所見と検査所見の時期が5年以上あるいは12週未満の場合は除外され，検査所見は12週以上の間隔を空けて2回陽性であることが必要である．

　一方，今回の「分類基準」に含まれていない抗リン脂質抗体も多数存在する（表2）．日本産科婦人科学会の2004年生殖内分泌委員会報告^2）では，表2に示す抗リン脂質抗体を推奨する検査としている．したがって，抗リン脂質抗体陽性と抗リン脂質抗体症候群の習慣流産・不育症は異なることになる．

　本稿では，新しい抗リン脂質抗体症候群の「分類基準」の原著論文^1），2004年生殖内分泌委員会報告^2）と電子教科書であるUp To Date^{3, 4）}やCochrane Database of Systematic Reviews^5）を参考に，治療法の解説をする．
