6.薄い子宮内膜を発育させるには？

1 はじめに

　近年，体外受精胚移植法（in vitro fertilization-embryo transfer : IVF-ET）などの生殖補助医療技術（assisted reproductive technology : ART）の進歩は，多くの難治性の不妊カップルに福音をもたらしてきた．最近の日本産科婦人科学会の報告（2005年）^1）によると，IVF-ETで新鮮胚を用いた成績では採卵成功率94.5％と良好であるが，移植当たりの妊娠率は30.4％にとどまっており，胚移植に対して着床障害が想定されている．着床率・妊娠率を向上させるには，着床障害に対して新たな視点からの診断・治療が望まれている．

　着床障害への治療は，3種の病態，すなわち胚因子，卵巣黄体機能因子，子宮因子の異常に対する対応である．本稿では，着床障害に対する治療として子宮因子，特に「薄い子宮内膜」に関しての病態，およびそれらへの対応策について概説する．
