2.子宮頸癌における術前化学療法の適応は？

1 はじめに

　局所進行子宮頸癌に対する術前化学療法（neoadjuvant chemotherapy : NAC）は，（1）腫瘍縮小に伴う手術完遂度および適応症例の増加，（2）潜在・微小転移巣の制御，（3）縮小手術の可能性，などを目的として種々の臨床研究が行われてきた．近年，有効な抗癌剤の導入により卵巣癌に準じた奏効率が得られ，メタアナリシスでも一部に有用性が報告されている．しかしながら，NAC導入後約20年が経過した現時点で，NAC後の手術の根治性と機能温存，術後治療などの重要な臨床事項は依然不明であり，標準的治療には至っていない．今後，早急に解決すべき課題は以下の4点であろう．（1）対象，（2）推奨レジメン，適切なサイクル数，（3）同時化学放射線療法（concurrent chemoradiotherapy : CCRT）との比較試験，（4）組織型別効果（腺癌）の研究．

　これまでは小規模な第2相試験やランダム化比較試験（randomized controlled trial : RCT）が多く，対象やレジメンも異なることより，十分なエビデンスが確立できない．特に欧米からの研究・報告が些少なことが大きな原因の1つと考えられる．これは局所進行症例に対する治療方針に関して，手術療法が中心の本邦と，放射線治療が中心の欧米との歴史的な治療スタンスの相違に加え，近年の欧米でのCCRTの標準化が大きく影響している．これらを踏まえ，本稿では子宮頸癌に対するNACに関して，その現状と今後の展望を中心に概説する．
