3.子宮体癌腹腔細胞診陽性は予後因子か？

1 はじめに

　1988年，国際産科婦人科連合（Federation International de Gynecologie et Obstetrique : FIGO）が子宮体癌の進行期分類を改訂し^1），腹腔洗浄細胞診が陽性の場合は手術進行期IIIa期に分類されることとなった．子宮体癌の大部分はI期であるため，III期は「進行癌」と認識されることが多く，ほとんどの施設で術後補助療法を追加しているのが現状であろう．

　一方，III期体癌のなかでは，腹腔洗浄細胞診陽性のみの例は，比較的予後がよいことが知られており，III期体癌のなかでも別扱いすべきであるという意見もある．2006年に発行された日本婦人科腫瘍学会編「子宮体癌治療ガイドライン」^2）でも，CQ12「腹腔細胞診陽性は独立した予後不良因子か？」という1項を設けており，さらに術後再発リスク分類で腹腔洗浄細胞診陽性は高リスクではなく，中リスクに分類されている．

　このように腹腔洗浄細胞診陽性については，理解と取り扱いについて変遷があり，必ずしも合意が得られているわけではないので，本稿で整理してみたい．
