融解胚移植における子宮内膜の管理

はじめに

　現在，日本産婦人科学会では多胎妊娠を避けるために，ARTにおける移植胚数は3個以内と定めているが，培養技術の進歩によりさらに少なくなる傾向にある．生じる余剰胚は，凍結融解胚移植の応用により採卵周期以外にも移植可能になっただけでなく，患者に対する経済的，肉体的負担や，卵巣過剰刺激症候群の発生率を減少させることができることから，凍結融解胚移植は臨床上必要不可欠な技術となっている．凍結方法は，従来，プログラムフリーザを用いたslow freezing法（緩慢凍結法）が一般的に行われていたが，最近では，より簡便なvitrification法（超急速ガラス化法）が行われ，妊娠例が報告されるようになった．手技の向上とともに凍結融解胚の生存率が70％前後と高くなっており，移植周期当たりの妊娠率は新鮮胚移植周期の妊娠率と差を認めないとする報告が多い^1）．そこで，本稿では，凍結融解胚移植における子宮内膜の管理について，当院での成績と文献的考察を加えて述べる．
