1.卵管妊娠

卵管妊娠の疫学

　子宮外妊娠は妊娠のおおよそ1%と報告され，そのうち卵管妊娠の占める割合は，自然妊娠において98.3%，生殖補助技術（assisted reproductive technology : ART）後の妊娠において82.2%である^1）．感染，炎症，外科手術の既往などが危険因子となり，卵管に障害が生じる．卵管内腔および卵管采の障害によるものが大半を占め，自然妊娠における子宮外妊娠は膨大部が79.6%，卵管采が6.2%であるのに対し，ART後の子宮外妊娠では膨大部が92.7%に及ぶ^1）．独立した子宮外妊娠の危険因子としては，次の項目が挙げられる^2）．

1. 骨盤内炎症性疾患（PID）の既往

　クラミジア腹膜炎は最も重要な起炎菌であり，卵管妊娠の7～30%で認められると報告されている^{3, 4）}．

2. 子宮外妊娠既往

　卵管妊娠の保存手術後の子宮外妊娠反復率は13%である．

3. 経口避妊薬と子宮内避妊用具（IUD）使用者

　経口避妊薬服用既往のある者の子宮外妊娠のリスクは上昇しない^5）．IUDはリスク因子とされてきたが^6），最近の知見ではIUDは子宮外妊娠のリスクは上昇させないとのレビューもある^7）．

4. 卵管形成術後

　総じて卵管形成術後の子宮外妊娠発症率は2～7%と報告されている．
