生殖医療と遺伝カウンセリング

はじめに

　生殖医療は配偶子形成～受精～着床～胚の発育という過程を取り扱うことから，本質的に遺伝医療と密接な関係を有している．しかし，以前の不妊治療は，排卵調節や人工授精，卵管形成などが中心で，技術的に受精現象そのものを扱うわけではなく，遺伝医療との接点は少なかった．その転機となるのが生殖補助技術（assisted reproductive technology : ART）の導入で，1978年に世界で初めて児が誕生した体外受精である．本来ならばこの時点で，遺伝カウンセリングが生殖医療にも取り入れられるべきであったと考えるが，当時の日本においてはまだそうした概念が周知されていなかった．その後の顕微授精や着床前診断の導入に伴い遺伝カウンセリングの必要性が認知されてくることになる．

　生殖医療における遺伝カウンセリングは専門誌の特集号として，いくつも参考になるものが刊行されている^1～3）．本稿では特集のタイトル通り生殖医療における遺伝カウンセリングのいくつかのトピックスに限定して記載した．
