1.婦人科がん手術における卵巣温存

卵巣摘出のもたらす影響

　本邦における悪性疾患の罹患数は増加の一途をたどっており，若年者の罹患数も増加している．一般的に固形腫瘍においては，原発巣の切除が治療の原則であるが，婦人科で取り扱う臓器は直接妊孕性と関係しており，根治的切除は妊孕性の喪失を意味する．また，性成熟期女性において卵巣の摘出は，妊孕性の喪失に留まらず，エストロゲン欠乏に伴うのぼせ，発汗，性交障害，うつといった卵巣欠落症状，将来の骨粗鬆症や高脂血症に伴う虚血性心疾患などの悪影響をもたらす．術後のQOLを維持するためには卵巣の温存が必要であるが，治療成績を下げることなくQOLの維持が可能であるかどうかを十分に吟味する必要がある．本稿では，代表的な婦人科悪性腫瘍（子宮頸がん，子宮体がん，卵巣がん）における最近の文献，および治療ガイドラインより，卵巣温存の適応に関して考察してみたい．
