8.婦人科がんの内視鏡下手術

はじめに

　現在わが国において腹腔鏡手術は主に付属器腫瘍，子宮筋腫，子宮内膜症などの良性疾患を対象として施行されている．しかし世界的にはすでに20年以上も前から悪性疾患に対する導入が試みられており，最近ではその適応がさらに拡大する傾向を見せている．内視鏡下手術では術後回復が早いため患者のQOLの改善が期待できるのみならず，後療法も速やかに行えるが，この利点を生かすためには開腹手術と同等の根治性を確保することが大前提となる．近年の内視鏡下手術手技および手術器具の進歩は，従来非常に困難と思われていた高度な手術を可能としつつあるといえるが，その一方でそれらの手技はやはりまだ一般的とは言い難く，一部のエキスパートによって行われていると言わざるを得ない．本稿では婦人科癌に対する内視鏡手術の歴史，および現況について記載する．
