5.多囊胞性卵巣症候群の卵巣所見はどのように判定するのか，国内外のデータに基づいて教えてください．また，臨床的な指標で，卵巣所見と相関しているものは何かありますか．

[1]はじめに

　多囊胞性卵巣症候群（polycystic ovary syndrome:PCOS）は，卵巣の多囊胞性変化に内分泌学的異常，およびその結果としてのさまざまな臨床症状を伴った複雑な病態を呈する内分泌疾患であり，不妊外来や一般婦人科内分泌外来でよく遭遇する疾患である．PCOSの卵巣形態は表面が凸凹に隆起した卵巣の腫大と白膜の肥厚を伴い，被膜下に長径10mm未満の閉鎖卵胞が多数存在し，組織学的には，間質細胞の増生，内莢膜細胞の過形成と顆粒膜細胞の変性萎縮が認められる（図1）．近年，超音波診断装置の発達により，PCOSの卵巣形態が経腟超音波で非侵襲的に観察が可能となり（図2），2003年のRotterdam Consensus Meetingでの新しいPCOSの診断基準（ESHRE／ASRM 2003）^1）および2007年に作成された本邦におけるPCOSの診断基準^2）においても経腟超音波検査における卵巣所見が採用され，卵巣の小卵胞のカウントルールが明確に記載された．

　本稿では，経腟超音波検査法を用いたPCOSの卵巣形態の判定所見，および卵巣所見と関連する内分泌指標について解説する．
