[1]はじめに

　1990年代半ばまでは，アメリカ生殖内分泌医の89％は不妊初期検査として腹腔鏡検査を行ってきた^1）．子宮卵管造影（hysterosalpingography:HSG）で卵管異常が認められた場合は速やかに，正常の場合は最終確認の意味で腹腔鏡検査が行われていた．

　しかし，95年以降は体外受精胚移植（in vitro fertilization:IVF）の妊娠率が20％となり，卵管性不妊に果たす腹腔鏡の役割は急激に縮小した．ASRM2006の報告^2）は，「不妊検査おける腹腔鏡検査の対象は，子宮内膜症や骨盤付属器癒着の証拠があるか強く疑われるときに行う．あるいは，明らかな卵管病変があるときも行う」とし，腹腔鏡の適応範囲は狭くなった．本稿では，腹腔鏡検査の導入時期の原則と各論を述べる．
