47.IVF周期前のカウフマン療法において，低用量ピルを用いた場合のメリットとデメリットについて教えてください．

　症例によって刺激前の卵巣環境は異なるため，それぞれの環境をよく理解し，できるだけ良好な胞状卵胞（antral follicle）を揃えておく必要がある．特に最近では，刺激周期前に低用量ピルを1～2周期服用することにより閉鎖不全の卵胞遺残が減少し，良好胚が得られる確率が上昇するとの報告がされており，月経不順，過去に良好胚を得られなかった症例では考慮すべき方法である．しかし，その周期数あるいは低用量ピルの種類，ゴナドトロピン投与開始時期については，まだ意見が分かれている．

　現在最も多く用いられている方法は，一層性の低用量ピルを刺激前周期に14～28日間投与し，FSH値を低下させてから刺激周期に入るものである．低用量ピルを投与することによって，内因性FSH値とLH値が抑制され，baseline ovarian cystの形成が抑制されること，妊娠周期にGnRHアゴニストを使用する心配がないこと，胞状卵胞のサイズが均一化すること，ある程度採卵日を調整することができることなどの利点が挙げられる．Baseline ovarian cystはGnRHアゴニストのflare upによるものであるが，これが存在すると卵巣刺激の効果や妊娠率などが低下するという報告もあれば，関連がないとする報告もある^{1, 2）}．低用量ピルとGnRHアゴニストは5日間ほどオーバーラップして使用する．
