53.インスリン抵抗性改善薬をインスリン抵抗性のない痩せた多嚢胞性卵巣症候群症例や多嚢胞卵巣のみのhigh responder症例へ投与した場合の有効性について教えてください（本当に効果があるのでしょうか）．

[1]非肥満多嚢胞性卵巣症候群にインスリン抵抗性改善薬は有効か

　欧米の多嚢胞性卵巣症候群（polycystic ovary syndrome:PCOS）はインスリン抵抗性を有する肥満例が多く，インスリン抵抗性改善薬が用いられてきた．しかしながら，日本のPCOSは非肥満例が8割を占め，非肥満例にインスリン抵抗性改善薬が効果的かは重要課題である．非肥満PCOSにメトホルミンを投与した報告は少数であるが，それらの報告のなかで非肥満またはやせと称している群の治療成績を抜粋して表1にまとめた^1～12）．また，作用がメトホルミンよりも強いチアゾリジン系のインスリン抵抗性改善薬であるピオグリタゾン，ロシグリタゾン^7）には，メトホルミンと同等またはそれ以上の排卵誘発作用があると推測されるが，まだ報告が少ないので今回は省略する．

　Ngら^1）はクロミフェン抵抗性のPCOSに限定して，中国人のPCOSにメトホルミンを3か月間投与した．BMIは17.9～30.8と非肥満を含む対象で，メトホルミンの排卵誘発効果はまったくみられなかったと報告している．それ以外の報告は，メトホルミンの投与で排卵誘発または正常月経回復の効果が得られたとしているものが多く^2～11），高度の肥満群よりもむしろ非肥満群が奏効しやすいと述べている報告もある^{2, 4）}．しかしながら，海外の論文では，BMIが25を超え，日本では肥満例に該当する（表2）症例が，非肥満ややせとして取り扱われていることが多いことに注意が必要である．
