[1]はじめに

　多嚢胞性卵巣症候群（polycystic ovary syndrom:PCOS）は月経異常，不妊，多毛，肥満などの特有の臨床症状と，卵巣の多嚢胞性変化，血中LH≧FSHや高アンドロゲン血症などの内分泌異常を主徴とする疾患である．1935年，SteinとLeventhalによってはじめて唱えられ，卵巣の部分切除術後に正常月経周期の回復と妊娠の成立が報告された．以後楔状切除として確立されてきたが，効果が一時的なことや術後癒着をきたす欠点があった．1984年，腹腔鏡下手術による卵巣焼灼術が報告され，近年の腹腔鏡手術の発展とともに卵巣楔状切除術に代わって腹腔鏡下卵巣多孔術（laparoscopic ovarian drilling:LOD）が盛んに行われるようになった．本稿ではLODの適応，成績とその手技の実際について述べる．なお，PCOSの診断基準，薬物療法の詳細については他稿を参照されたい．
