61.不妊症で子宮内膜ポリープ，粘膜下子宮筋腫がある場合，子宮鏡下の病巣摘出と不妊治療のどちらを優先すべきでしょうか．具体的に教えてください．

[1]はじめに

　子宮筋腫と子宮内膜ポリープは不妊治療を行っていれば必ず出会う疾病である．表1に当科通院中の不妊症例において超音波検査にて子宮内膜異常を認めた症例（478例）に対して行った子宮鏡（hysterofiberscopy:HFS）の結果を示す．

　HFSで子宮内膜ポリープを認めた場合，切除すべきか，経過観察でもよいのかの判断は難しい．子宮筋腫に関しても大きな漿膜下・筋層内筋腫や粘膜下筋腫の症例や，少しでも早く体外受精をしたい高齢の筋腫合併患者に対し，手術を優先すべきか一般不妊治療や体外受精を優先すべきか悩むところである．実際には明確な指針はないのが現状であり，本章ではいくつかの文献を示し，その取り扱いについて考察したいと思う．
