70.ゴナドトロピン反応不良例への調節卵巣刺激法について教えてください．

　最近の平均初婚年齢および初産年齢の上昇に伴い，挙児希望年齢も高まりをみせている．さらに第二次ベビーブーム世代が30歳台後半を迎える現在，比較的高齢になってから挙児希望を主訴に訪れるカップルが増加している．妊孕能は30歳頃から次第に低下することが知られており，35～40歳をすぎると急激に低下し，一般には41歳が妊孕性の終了時期と考えられる^1）．このような患者においては卵巣の反応性も低下していることが多く，排卵誘発治療に苦慮することも多い．また，若年者においても排卵に至るまでに非常に多量のhMG製剤を必要とするものや，きわめて少数の卵胞発育しか認められない，いわゆるpoor responderが存在しており，これらの症例の卵巣予備能を知ることが反応性を予測するうえで有用である．
