今月の臨床 HRTの新ガイドラインを読み解く

はじめに
閉経や両側卵巣摘出術によってエストロゲン分泌の欠乏した女性に，エストロゲンを中心とした女性ホルモンを補充するホルモン補充療法（hormone replacement therapy : HRT）は，エストロゲン欠乏に伴う諸症状や疾患の予防ないし治療に対して合目的的であり，20世紀後半まで閉経後女性の健康維持や改善に有用であるとして普及してきた1）．
しかし，HRTも平坦な道を辿ってきたわけではない．1980年代前半にはエストロゲンは子宮内膜癌のリスクを高めるということで，HRTは下火となったが，黄体ホルモン（progesterone）を併用することによってほぼ解消された．ところが，乳癌リスクも上昇することが1980年代後半に判明し，この点に関しては黄体ホルモンを併用することによっても解消されず，長い間，乳癌はHRTのリスクとして危惧されてきた．エストロゲンの効果および有用性は十分わかっていたが，これらの子宮癌と乳癌のリスクがないエストロゲン製剤が待ち望まれていたわけである．そこで登場したのが分子薬理学的手法を取り入れることによって創薬に成功したSERM（selective estrogen receptor modulator）である2）．その結果，第一世代のタモキシフェンが乳癌の予防・治療薬として，また第二世代のラロキシフェンが骨粗鬆症の予防・治療薬として，それぞれその役割を担っている．
以上とは別に，HRTのリスクとベネフィットをランダム化比較試験（randomized controlled trial : RCT）により明らかにしたものがWHI（Women's Health Initiative）3）であり，また乳癌リスクについて言及したのが観察研究ではあるがMWS（Million Women's Study）4）である．
そこで，ここではWHIを中心にHRTのメリットとデメリットについて整理してみたい．ただし，HRTのメリットとデメリットについては評価が非常に難しく，どのような対象にどのような内容のHRTをどのくらいの期間使用したかによってメリットとデメリットは微妙に変わる5）ことをまずお断りしておきたい．
