1.不整脈の胎内治療

はじめに

　現在，種々の胎児疾患に対する胎児治療が行われ始めているが，有効な治療法と証明されているものはいまだ少ない．そのなかで胎児不整脈は，早くから胎児治療が可能な疾患として認識され治療法が普及している疾患の1つである．

　1980年代より，超音波診断装置の進歩に伴い胎児不整脈の胎児診断が急速に普及した．当初より，胎児頻脈に対する抗不整脈薬による胎児治療が報告され，その有効性が証明されている．近年では，さらに多施設からの報告に基づき，より有効な胎内治療法の研究が進められている．胎児徐脈に対しても，近年，有効な治療法や周産期の管理法が報告され，予後が改善されつつある．

　本邦においては，通常の妊婦検診時に胎児心拍モニターあるいは胎児エコーを施行することが広く普及しており，胎児不整脈が診断されその管理を行う機会が増加してきている．出生前診断が可能となった現在，胎児不整脈の予後の改善のため，より詳細な診断と有効な治療法や管理法の研究が注目されている．

　本稿では，まず胎児不整脈の診断について述べ，胎児頻脈，胎児徐脈，胎児期外収縮それぞれにおける現時点での胎内治療法と管理法およびその予後に関して解説する．
