4.子宮頸がん検診とHPV-DNA検査

子宮がん検診の現状と問題点

　昭和57年の老人保健法制定以来，全国的に開始された細胞診による子宮頸がん検診は，子宮癌の発生率の低下のみならず日本にがん検診を定着させた指導的役割はきわめて大きい．厚労省も「検診による死亡率減少化効果がある」と評価した．しかし，国の政策転換によりがん検診事業は平成10年4月からは地方行政の任意事業となった．さらに，平成15年4月には「がん予防重点健康教育及びがん検診実施のための指針」で検診間隔を2年とした．このような状況のなかで，わが国の子宮がん検診は以下のような問題を抱えている．

　（1）検診受診率が20％前後と低迷している．特に新規の受診者や20歳代での受診がきわめて少ない．

　（2）検診における細胞診の精度管理が求められている．

　（3）子宮頸部癌の発生率は低下したが，最近では特に生殖年齢層に罹患率・死亡数が増加している．

　（4）性交開始の低年齢化と性交渉の多数化・多様化が生じ，子宮頸癌の原因ウイルスであるHPV感染が若年者に増えている．

　（5）わが国の経済が逼迫するなかで医療経済学の立場から科学的で合理的な検診システムが求められている．

　今後，新たな英知を取り入れた科学的・合理的でかつ日本の状況に適した子宮がん検診システムを構築していく必要がある．
