はじめに

　多嚢胞性卵巣症候群（polycystic ovarian syndrome : PCOS）に対してFSH／hMG製剤によって卵胞発育を促し，成熟卵胞に対してhCGによる排卵誘発を行うゴナドトロピン療法は排卵率・妊娠率の点で有効な治療法であるが，卵巣過剰刺激症候群（ovarian hyperstimulation syndrome : OHSS）と多胎という副作用を常に意識する必要があり，厳密に100％多胎を防ぐことができるプロトコールはいまだ完成されていない．これはPCOの病態が多様であり，hMGに対する反応性や，患者により卵子1個当たりの妊孕性が異なっていることも関係している．しかし，プロトコール通りに排卵誘発を行うことによって，多胎の多くを回避することができることもまた事実である．

　さらに，近年わが国でも遺伝子組換えFSH（recombinant FSH，以下recFSH）製剤が保険収載となり，また同製剤の患者による自己注射が認可となった．このことにより，いままで患者の通院負担を考慮してどうしても確実に効果が出るやや多めの量を投与しがちであったこの治療において，患者の負担をそれほど増加させずに単一卵胞発育を起こすことができるようになってきている．

　そこで本稿では，これまでの報告に基づいて，基本的な単一卵胞発育のための低用量FSH漸増療法のプロトコールの理論と実際を解説するとともに，このプロトコールを施行するために必要なrecFSH製剤とその自己注射について紹介する．

　本文中，特に断りなく「FSH」と書いてある場合，hMG，尿由来FSH，recFSHのすべてを意味することとする．また，特に断りなければ「卵胞径」は平均卵胞径の意味である．低用量FSH漸増療法については，ご存じのようにHomburgらの優れた総説^1）や，最近ではESHRE consensus-groupの報告^2）がある．本稿も多くの部分これを参考としているので，興味のある読者はぜひ参照されたい．
