中大脳動脈(middle cerebral artery : MCA)の血流
MCAは脳を還流する血流を支配する重要な血管で,内頸動脈から分枝し,シルビウス裂溝の部位を含む皮質・皮質下の組織,被殻や淡蒼球を支配している.胎児超音波検査における血流評価においてMCAはその解剖学的特徴から汎用されている.MCAの描出は,胎児発育の評価に用いる児頭大横径の計測断面のやや下方でウィルス動脈輪とシルビウス裂溝を含む水平断において可能である.血流波形の解析においてはresistance index(RI)やpulsatility index(PI)による血流抵抗の評価はほかの血管と同様可能であるが,Doppler beamの血流方向への入射角を0度とすることが容易なため血流速度の絶対値による評価が可能で,特に最高血流速度(peak systolic velocity : PSV)は臨床検査として有用である.
