超音波治療

　超音波エネルギーの発熱作用は古くから知られており，そのエネルギーを利用した超音波温熱治療は，超音波の診断への応用に先駆け50年以上前からすでに臨床応用されている．超音波治療に関する論文数をPubMedで検索したところ，1950年から1969年の20年間ですでにおよそ1,000編報告されている．以後報告数は漸増しているが，2000年以降さらに増加し始め，ことに2005年以降は急増している．治療法もこの間変遷し，当初は温熱療法が主体であったが，最近では遺伝子導入などの分子的超音波療法の開発，強力集束超音波（high-intensity focused ultrasound : 以下，HIFU）などが急速に臨床応用されはじめた（図1）．

　超音波エネルギーは大きく熱作用と非熱作用に分けられ，それぞれにハイパワーとローパワーがり，熱作用のハイパワーがHIFUであり，非熱作用のハイパワーに超音波メスや結石破砕がある．熱作用のローパワーに温熱，物理療法が，非熱作用のローパワーに遺伝子導入，薬物効果促進などの分子超音波治療や血栓溶解治療などがそれぞれある（図2）．
