1．子宮頸癌の膀胱・直腸浸潤の判定

はじめに

　子宮頸癌の進行期は術前の診察および検査で決定される臨床的進行期であり，CT, MRI, PETなどの最新の画像診断を進行期決定に用いてはいけないことになっている（子宮頸癌取扱い規約）．ただしこれらの画像診断所見を治療方針決定の参考とすることは当然認められる．子宮頸癌の膀胱・直腸への浸潤は，予後推測や治療法選択のうえで非常に重要なポイントであり，治療前の評価で見逃してはならない．粘膜におよぶ浸潤は膀胱・直腸鏡により診断可能であるが，漿膜や筋層に留まる浸潤はMRIなどによる画像検査が必要となる．子宮頸癌の局所浸潤の評価法には，軟部組織のコントラスト分解能が高いMRIが最も有用で，研究報告も多い．本稿ではこれらの報告を基にMRIの膀胱・直腸浸潤の判定における精度や問題点について検討した．
