はじめに

　子宮内膜症では子宮内膜様組織が子宮内膜以外の卵巣，仙骨子宮靱帯，腹膜などに発生し，癒着とチョコレート嚢胞を形成する．このような病態が月経痛，慢性骨盤痛，排便痛や性交痛の疼痛症状，不妊症および嚢胞の癌化などを惹き起こす．子宮内膜症の治療には薬物療法と手術療法があるが，不妊症を合併する例や癌化の懸念がある場合は手術療法が選択される．その手術療法には根治療法（両側卵巣摘出）と保存療法があるが，月経のある成熟女性に多い疾患であるために卵巣を温存する保存療法（子宮内膜症病巣除去術）が選択されることが多い．また，アプローチの方法として開腹手術と腹腔鏡下手術があるが，最近では侵襲性や術後癒着の点から腹腔鏡下手術を選択されることが多い．ここではチョコレート嚢胞に対する卵巣機能を温存する外科治療としてチョコレート嚢胞内壁焼灼術を取り上げ，そのコツを文献的な考察を含めて述べる．
