4．肥満妊婦の周産期合併症

はじめに

　肥満は高血圧や心疾患，耐糖能障害，高脂血症のリスク因子である．肥満女性は，これらの病態を有していなくても，その発症リスクとなる身体的背景を有している頻度が高い．したがって，肥満妊婦では妊娠に伴う生理機能の変動にそれらのリスク因子が直接あるいは間接的に加わってさまざまな異常が発生しやすい．実際，肥満女性は適正体重の女性に比べ，妊娠から分娩にかけてより多くの医療サービスを必要としているとの研究成績も報告されており^1），社会医学的観点からも問題が多い．本稿では，肥満妊婦に関して論じられてきた広範囲の問題のなかで，特に周産期合併症に関して，分娩にかかわるもの以外について概説する．
