腸上皮型粘液性卵巣境界悪性腫瘍で妊孕性を温存した腹腔鏡下手術の1例

　上皮性境界悪性卵巣腫瘍は，術前には診断が難しく良性または悪性のどちらとも診断され得る疾患である．卵巣がん治療ガイドラインでは，卵巣癌に準じた治療が望ましいとしているが，予後が比較的に良好であるために，妊孕性を温存した手術の報告が増えてきている．今回われわれは，若年者の腸上皮型粘液性境界悪性腫瘍を経験したので，文献的考察とともに報告する．

　症例は34歳の未婚女性．下腹部痛を主訴に受診．精査により，良性の両側性卵巣嚢腫と診断し，腹腔鏡下卵巣嚢腫摘出術を施行した．術後病理組織検査から，腸上皮型粘液性境界悪性腫瘍Ic（b）および内膜症性嚢胞と診断された．そのため，追加治療として腹腔鏡下に患側の付属器切除術を施行した．現在，外来にて経過を観察しており，術後10か月経過したがこれまで再発は認めていない．
