7．腟式広汎子宮頸部摘出術

術式の特徴とストラテジー

　進行子宮頸癌患者に対する妊孕能温存手術として広汎子宮頸部摘出術（radical trachelectomy）があり，この術式による治療成績などが1990年代以降報告されている^{1, 2）}．この術式は，子宮頸部と傍子宮結合織を広汎子宮全摘出術と同じ切除範囲で摘出することで根治性を保ちつつ子宮体部を温存する術式である．腟式の場合は，頸部摘出術に先立ち，腹腔鏡下で骨盤リンパ節郭清術を行うのが一般的である．

　わが国でも浸潤子宮頸癌症例に対する子宮温存治療の選択肢の1つとして行われており，腟式手術後の周産期予後についての報告^3）や61例に及ぶ腹式手術の報告^4）もなされている．しかし，癌に対する根治性，術後管理，妊娠した場合の周産期管理などコンセンサスが得られていない面も多く，手術の適応については慎重な判断が必要である^5）．
