はじめに

　深部静脈血栓症（deep vein thrombosis : DVT）は肺血栓塞栓症（pulmonary embolism : PE）の発症に深く関連しており，両疾患は静脈血栓塞栓症（venous thromboembolism : VTE）に含まれる連続した1つの病態として理解し，深部静脈血栓症を適切に管理することが重要である．四肢の静脈には筋膜より浅い表在静脈と深い深部静脈があり，急性の静脈血栓症は深部静脈の深部静脈血栓症と表在静脈の血栓性静脈炎に区別される．深部静脈血栓症は，発生部位（頸部・上肢静脈，上大静脈，下大静脈，骨盤・下肢静脈）により症状が異なる^1）．上肢や腸間膜のような稀な部位の深部静脈血栓症は全体の10％未満に過ぎず^2），欧米では発生頻度の高い下肢の深部静脈に発生するものを深部静脈血栓症としている．本稿では症例の多い下肢静脈の深部静脈血栓症の診断について解説する．
