静脈血栓塞栓症に対する抗凝固療法

はじめに

　抗凝固療法は，静脈血栓塞栓症の基本的治療法であり，原則として禁忌例を除く全症例に用いられる．わが国で治療薬として承認されている薬剤は，主に急性期に用いられる未分画へパリンと急性期から慢性期にかけて用いられるワルファリンのみであるが，今後は欧米ですでに承認されている新しい薬剤がわが国でも使用可能となることが期待されている．治療目的で使用する抗凝固薬は，肺動脈や深部静脈に存在する血栓への二次血栓形成を抑制するとともに内因性線溶による血栓溶解を促すことになる．また，静脈血栓塞栓症の一次予防においても，高リスク例に対しては抗凝固薬を用いた薬物予防が推奨されている．予防目的では，従来の未分画へパリン，ワルファリンに加えて，腹部外科や整形外科領域の手術時にフォンダパリヌクスやエノキサパリンが使用可能である．本稿では，静脈血栓塞栓症に対する治療・予防における抗凝固薬の使い方，効果，合併症などの現状について解説する．
