8．ターナー症候群

1 概　念

　ターナー症候群（Turner's syndrome : TS）は，1938年，Henry Turnerが性腺発育不全を伴う低身長，翼状頸，外反肘などの特徴を持つ女性7例を新たな症候群として報告したことに始まる^1）．その後，染色体の解析が可能になり，1959年にこの疾患が性染色体の欠損（45,X）であることが判明した^2）．性染色体分析の研究が進み，後述するようにTSには多くの性染色体構造異常やモザイク例などのバリエーションが存在することが明らかとなった^3）．さらにTS症状の責任遺伝子として，成長障害およびTS骨格徴候のSHOX ^4）などが同定され，短椀が完全欠失すればpairing failureのため性腺異形成，SHOX欠失のため低身長，骨格徴候を示すことが理解できるようになった．しかしながらいまだTSの定義にはあいまいさが残り，現時点では「X染色体欠損症が存在しTS徴候を認めるもの，あるいは徴候がなくても明らかな短椀欠失を認めるもの」とされるが，今後は，TSの臨床的特徴があり，核型レベルで欠失がなくても遺伝子レベルで欠失が証明されればTSの範疇に入れられるものと思われる．
