1 概　念

　アンドロゲン不応症（androgen insensitivity syndrome : AIS）は，以前は精巣性女性化症候群（testicular feminization syndrome）といわれていた男性仮性半陰陽の1つである．現在はアンドロゲン受容体の遺伝的な異常に起因した病態であることがわかっており，アンドロゲン不応症といわれることが多い．全く男性化の認められない完全型と，部分的な男性化を伴う不完全型に分類される^1）．染色体が46,XYであるにもかかわらず表現型が女性であるAISでは，生物学的な性（sex）と社会的な性（gender）に乖離がみられ，その取り扱いには医療従事者の十分な病態の理解が必要である．

　完全型AISでは第2次性徴を経てから，原発性無月経を主訴に産婦人科を受診することが多い^{2, 3）}．その際に短く盲端に終わる腟，乏しい恥毛の発育，そして女性内性器の無形成などの所見で本症が疑われる．AISと診断された場合，悪性化のため精巣摘出を考慮する必要がある^4）．また，性腺摘出後はホルモン補充療法が必要になる．
