13．肥満と月経異常

　月経異常の原因は多岐にわたる．やせほどではないにしろ肥満によっても無月経に陥る．そして適正な体重に戻す（減量する）と月経が再来することはしばしば経験される．また，初経発来が体重や体脂肪量と密接に関係していることも知られている．これらの事実は性機能が体重と密接に関係していることを示している．現在は飽食の時代である．エネルギーの過剰摂取や運動不足から肥満になりやすい．したがって，肥満と関連した月経異常は，現代生活に根ざした文明病といえるかもしれない．

　肥満は月経異常以外にも，高血圧症，糖尿病，高脂血症，動脈硬化症などの内分泌・代謝異常を伴いやすい．しかし肥満の程度とこれらの内分泌代謝異常の発生頻度や重症度は必ずしも相関しない．肥満が体脂肪組織の過剰な蓄積であると定義すれば，その蓄積量の絶対量（肥満度）よりも蓄積部位の異常（体脂肪分布の異常）が月経異常などの内分泌・代謝異常と関連して重要であることが明らかになりつつある．上半身型（内臓脂肪型）体脂肪分布は内臓（腸間膜や大網）に脂肪が過剰に蓄積し，下半身型体脂肪分布に比較して月経異常，高脂血症，糖尿病，高血圧症が多い^1）．上半身型体脂肪分布と関連するこれらの内分泌・代謝異常は，インスリン抵抗性（インスリンに対する感受性の低下）を共通の基盤として病因論的に密接に関連する疾患として認識されるようになっている．
