1．子宮筋腫

1 概　念

　子宮筋腫は，子宮平滑筋細胞の単クローン性増生による良性腫瘍である．生殖年齢の女性に発生し，月経過多や過長月経／不正出血などの月経異常・頻尿や便秘など臓器圧迫による症状・下腹部痛や腰痛あるいは月経痛などの疼痛症状などをきたす．子宮筋腫に対する治療はこれらの症状を改善する目的で行う．不妊の原因であることが疑われる筋腫や悪性腫瘍の否定ができない筋腫も治療の対象となる．

　手術療法には子宮全摘術と筋腫核出術があり，アプローチ方法には開腹・腟式・腹腔鏡・子宮鏡などがある．また，筋腫の縮小を目的とした子宮動脈塞栓療法（uterine artery embolization : UAE）やMRガイド下集束超音波法（magnetic resonance-guided focused ultrasonography : MRgFUS）も行われることがある．GnRHアゴニスト（GnRHa）は，副作用の面から長期投与が認められていない．
