2．インスリン抵抗性の変化

はじめに

　インスリン抵抗性とは，インスリンは分泌されているが，糖代謝が十分に行われていない状態のことである．インスリン抵抗性の発現には胎盤から産生されるホルモンやアディポサイトカインなどの関与していることが知られている．妊婦では妊娠の進行に伴って脂質蓄積（TNF─αの増加，アディポネクチンの低下）やインスリン拮抗ホルモンの増加がインスリン抵抗性を増すと考えられる．本稿では，インスリン抵抗性の評価および妊娠中の変化について解説する．
