4．MD-TESEの現状と課題

無精子症

　子どもを授からない場合，子どもが産めない女性に原因があるかのように以前は思われがちであった．しかし，不妊症の原因が男性側のみに存在するもの（約20％）と男女ともに存在するもの（約30％）を合わせて，少なくとも約半数に男性側の因子が関与していることが次第に知られるようになり，現在では，不妊症の検査や治療は男女ともに重要であると認識されている．男性の不妊症の原因は約9割が造精機能障害であり，その最も重篤な表現型が非閉塞性無精子症（non-obstructive azoospermia：NOA）である．すなわち，精巣内の精細胞において，精祖細胞から精子にまで分化する過程で障害をきたしたものである．先天性のものが多いとされ，従来，絶対不妊症と考えられてきた性染色体異常のKlinefelter症候群はNOAの約10％程度を占めるといわれている．

　NOAの診断において，以前は精巣生検術が行われていた．同時に精巣生検術で得られた精巣組織内に精子を認めず，NOAと診断されれば挙児の可能性がないものと判断されていた．しかし，やがて精巣組織は精細管部位により造精機能が不均一であること，同時にごくわずかな精子が精巣内に存在したとしても，必ずしも精液内に精子が射出されるものではないという事実が知られるようになった．そのため，仮にNOAであっても精巣組織内にわずかながら精子が存在する可能性が強調され，顕微授精（intracytoplasmic sperm injection：ICSI）を含めた最近の補助生殖技術（assisted reproductive technology：ART）の進歩も相俟って，いかにして精巣内精子を確実に採取し，ICSIに供するかが焦点となった．
