1．進行卵巣癌の手術―Neo-adjuvant chemotherapyの有用性

背　景

　わが国では毎年7,000人以上が卵巣癌に罹患し，死者数は毎年4,000人を超えている^{1, 2）}．卵巣癌は骨盤内臓器のため自覚症状が乏しく約40～50％の症例がIII，IV期の進行した状態で発見される^1）．パクリタキセルの登場により卵巣癌の治療成績は向上したが，進行卵巣癌の5年生存率は25～37％と依然として低く，進行例では手術治療と化学療法を組み合わせた集学的治療が必要となる．

　卵巣癌の手術療法で最も重要な点は腫瘍の完全摘出をめざすことであり，その完遂度によって患者の予後は大きく左右される．しかし，進行卵巣癌において腹腔内の状況によっては完全摘出が困難な場合も少なくない．そのような背景から，腫瘍減量手術前に施行する術前化学療法（neoadjuvant chemotherapy : NAC）の有用性についての検討が進められてきた．このNACを行う前に組織型および腹腔内所見確認のための生検を目的とした手術を施行することもある．NACを行うことにより手術の完遂度を高めることの可能性とそれにより生存率改善効果がもたらされるか否かが議論の焦点である．
