妊娠高血圧症候群による肺水腫を合併した周産期心筋症の1例

症例

患者　25歳，1経妊0経産．

主訴　呼吸困難感．

既往歴　特記事項なし．

現病歴

　自然妊娠による2絨毛膜2羊膜性双胎であり，近医にて妊娠26週まで妊婦健診を受けていたが，特に異常を指摘されていなかった．妊娠26週5日に当院初診．初診時には血圧121／74 mmHgと高血圧を認めなかった．妊娠32週2日に血圧143／85 mmHgと高値を認め，自宅血圧測定の指導を行った．また，経腟超音波検査にて頸管長が22 mmと短縮しており，明らかな子宮収縮やfunnelingを認めなかったものの，双胎妊娠であることも考慮し，塩酸リトドリン15 mg／日の内服を開始した．妊娠33週3日より呼吸困難を自覚したため，翌日当科を受診した．
