症例

左卵巣腫瘍のautoamputationによると考えられた遊離卵巣腫瘍の1例

要旨

　症例は47歳で，子宮がん検診時に右卵巣腫瘍を指摘され受診した．超音波検査およびMRIで，子宮の右腹側に径8 cmの嚢胞性腫瘤を認め，右卵巣腫瘍の診断で腹腔鏡下に手術を施行した．手術時左卵巣は索状で，右卵巣は正常大であった．膀胱子宮窩右寄りの腫瘍は，腫瘍前壁と壁側腹膜に索状の癒着を認めたため，癒着を剝離し摘出した．腫瘍は茎捻転後にautoamputationにより遊離し，腹膜に癒着した左卵巣腫瘍と思われ，病理診断は成熟嚢胞性奇形腫であった．autoamputationした遊離卵巣腫瘍はまれであるが，卵巣腫瘍の鑑別診断として考慮する必要があると思われた．
