子宮筋腫を合併した妊娠子宮の骨盤腔内嵌頓の1例

症例

患者　36歳，初産婦．

主訴　腹痛．

既往歴

　2～3年前から子宮筋腫を指摘されている．妊娠前のMRI検査は後屈と左子宮壁内の筋腫を認めている（図1）．

現病歴

　初診時は，最終月経および経腟超音波検査から妊娠7週1日であった．子宮の左壁に9 cmのサイズの子宮筋腫を認めた（図1，2）．

　妊娠8週に入り，左側腹部から腰背部にかけての疼痛が出現した．左の腎盂にやや拡張像があった．子宮筋腫による圧迫と考えられ，右側臥位か四つん這いの姿勢を1日数回はとるようにアドバイスし，その後，症状は10日ほどで改善した．
