3．妊婦の感染症と早産

●妊娠20週までに細菌性腟症を評価・治療すること，および妊娠中の無症候性細菌尿に対する抗菌薬投与は，早産予防効果があるとされる．

●過去に早産の既往がある妊婦や単胎妊娠の子宮頸管長短縮症例に対する黄体ホルモン（プロゲステロン）製剤の筋注，あるいは腟錠投与は，早産予防に寄与すると考えられている．

●いったん切迫早産，頸管無力症などの診断で入院した自然早産ハイリスク症例に対して，入院当日に子宮内の感染の有無，感染菌の同定，および子宮内炎症の程度を評価することが可能となった．羊水穿刺が必要であるが，この迅速システムにより最適な抗菌薬選択が当日に可能となるため，今後の治療成績に期待がもてる．
