症例

卵巣原発の大細胞神経内分泌癌の1例

要旨

　症例は68歳．受診3か月前より腹部違和感を自覚し，1か月前に近医を受診したところ，左卵巣腫瘍を指摘され，精査加療を目的に当科紹介となった．画像所見からは，左卵巣の充実性腫瘍を認め，左卵巣癌の疑いで試験開腹術を施行した．左卵巣は鵞卵大に腫大し，周囲組織と強固に癒着していた．術後の病理組織診断において，腫瘍は中型から大型の異型の強い細胞からなり，核分裂像も多数認められた．免疫組織染色において神経内分泌マーカーであるsynaptophysin，chromogranin A，CD56，NSEなどが陽性であったため，左卵巣原発の大細胞神経内分泌癌（large cell neuroendocrine carcinoma : LCNEC），stageIIcと診断した．術後補助化学療法としてdose dense TC（パクリタキセル＋カルボプラチン）療法6クール施行した．術後12か月を経過し，再発を認めていない．
