無精子症患者での挙児手段

はじめに

　顕微授精（intracytoplasmic sperm injection : ICSI）技術^1）の確立・普及によって，男性不妊の多くの症例で授精，妊娠が可能となり，従来は絶対不妊とされていた無精子症においても精巣精子採取術（testicular sperm extraction : TESE）により得た精子を用いて挙児を得ることが可能となった．通常，射精障害や閉塞性無精子症に対してはconventional TESE（c-TESE）が，非閉塞性無精子症に対しては顕微鏡下精巣精子採取術（microdissection TESE : MD-TESE）が施行される．顕微授精が標準治療となりつつあるmodern ARTの時代においては，精子の供給が可能であれば挙児のチャンスがあるといえる．射精液中に精子が存在しない無精子症患者に対する治療としては，男性不妊治療としての泌尿器科的アプローチが大きな役割を担ってくる．本稿では，無精子症患者に対するマネジメントについて，診断および治療を中心に解説する．
