両側肺転移に対し肺部分切除が有効であった子宮頸部明細胞腺癌の1例

要約

　患者は53歳，2経妊2経産，不正性器出血を主訴に受診され，子宮頸部に易出血性の不整形腫瘍を認め，子宮頸部腺癌IIA1期の診断にて広汎子宮全摘出術を施行した．術後病理検査で，子宮頸部明細胞腺癌pT2a1N0M0，断端陰性と診断した．術後10か月時に両側肺転移を認めたため胸腔鏡下肺部分切除を施行した．現在，肺手術後16か月経過しているが再発兆候はない．本症例では，転移の診断後すみやかに転移病巣を完全摘出したことにより，無病生存に至ったものと考える．子宮頸部明細胞腺癌は，放射線療法や化学療法が有効ではない場合が多く，再発・転移に対してはすみやかに摘出することで予後は改善される可能性があることが示唆された．
