常位胎盤早期剝離後の子宮動脈塞栓術により子宮小腸瘻を発症した1例

要約

　妊産婦死亡の主要原因の1つである分娩後出血への有用な治療法として，子宮動脈塞栓術（uterine artery embolization : UAE）が普及している．われわれは，常位胎盤早期剝離後の子宮出血に対するUAE後に子宮感染症が発生し，抗菌薬にて感染は制御できたものの，続発した子宮小腸瘻のため子宮摘出および小腸（回腸）部分切除に至った症例を経験した．産科出血後のUAEは外科的な子宮摘出に代わり得る非常に有用な治療法ではあるが，このような重篤な合併症が起きる可能性にも留意すべきである．
