5.子宮頸部円錐切除後の早産発症と予防

●円錐切除術後の早産発生機序としては,上行性感染に対する防御機構の破綻,子宮頸管の構造上の脆弱化,瘢痕形成による影響が考えられる.

●手術方法や切除範囲,妊娠までの期間などにより,早産発症のリスクは無視できるレベルから深刻なものまでさまざまである.

●円錐切除後妊娠の周産期管理に際しては,既往手術に関する正確な情報の取得に努め,適切な情報提供,治療選択および出産までのサポートを行うことが重要である.
